Filling Out PDF's for FNU

If you have had trouble filling out PDf forms or if credentialing has informed you that your filled out documents are appearing blank, then you may not be filling out the form properly.

Windows & Mac

Step 1. Make sure that you have Adobe Reader installed to your computer.

Step 2. Once you have Adobe Reader installed, you are ready to fill out the form. You will need to download a blank version of the document you are going to fill out. Often times when you download a PDF and open it, your browser will open the PDF in a browser window. If you fill out the form in the browser window, it will appear blank to anyone you send the document to. Instead, what you want to do after downloading the document, is to open the folder location of the document. Normally, this will be your Downloads folder. Once you locate the document in your Downloads folder, open it, and if Adobe Reader is installed it should open in Adobe Reader. Fill out the form and send. 

 

Mac Only

If you are using a Mac and you are not filling out the form in the web browser, then you are likely filling out the form using Preview. Preview is Apple's default PDF handler application. If you fill out a PDf using Preview, it will appear blank to FNU Staff. 

Step 1. Make sure you have Adobe Reader installed.

Step 2. Once you have Adobe Reader installed, download the document you need to fill out. Once the document is downloaded, open your downloads folder and locate the file. 

 

Control + Click the file and a sub-menu should appear.

Select Get Info

You should see an option that says "Open with:"

Select Adobe Reader in the dropdown.

Close the window.

Now, attempting to open the document should open it in Adobe Reader. Fill out the form in Adobe Reader and the form should not have any issues.
